I certify that my travel is related to the following reason (check the box) authorized under the general measures necessary to address the COVID-19 epidemic within the framework of the health emergency state:
Travel between home and the place of professional activity or education and training, professional travel that cannot be postponed;
Travel for consultations, examinations, preventive measures (including vaccinations) and healthcare that cannot be provided remotely and cannot be postponed or for the purchase of health products;
Travel for pressing family reasons, for assistance to vulnerable or precarious individuals, or for childcare;
Travel for persons with disabilities and their accompanying persons;
Travel to respond to a judicial or administrative summons;
Travel to participate in public interest missions at the request of the administrative authority;
Travel related to rail, air, or bus transit for long-distance travel;
Short trips, within a maximum one-kilometer radius around the home for pet needs.
